Custom Courtesy Cards

 

To order our 2025 Custom Courtesy Cards online, follow these directions.

1. 1. Download the 2025 Courtesy Card Order Form by clicking the link below
   2. Print and complete the form, mail it back with payment OR– skip to section 5 to order online
   3. If mailing your form back and would like to pay by credit or debit card click the payment page below.  if you choose this payment option be sure to add in the notes section that the form is in the mail
   4. If paying online, Enter the amount due based on the quantity ordered. 10 card minimum @ $25,00  Additional cards can be ordered in lots of 5 @ $2.50 for each additional card.
   5. To order online WITHOUT the printed form—-Click the payment page link below and—–Complete the NOTES section to spell out EXACTLY what you want on your card and verify the quantity ordered.  There is a 35 character limit.  Be sure to proof read your order and include your mailing address. What you request is what you get.  It is your responsibility to make sure it is correct.
   6. If you choose to use the paper form: Return the form either by US Mail… OR Scan it and send to secretarytreasurer@foplodge862.com
   7. IMPORTANT NOTE:  YOU will  receive an emailed receipt of payment.
   8. Cards will only be ordered IF YOUR 2025 Dues HAVE BEEN PAID
