Custom Courtesy Cards

 

To order our 2026 Custom Courtesy Cards online, follow these directions.

1. 1. Download the 2026 Courtesy Card Order Form by clicking the link below
   2. The form can be submitted by US Mail with a check or emailed to secretarytreasurer@foplodge862.com  If you choose the online payment option be sure to add in the notes section that the form is in the mail or being sent by email
   3. If paying online, Enter the amount due based on the quantity ordered. 10 card minimum @ $25,00  Additional cards can be ordered in lots of 5 @ $12.50 for each additional 5 cards.
   4. Online payments will  receive an emailed receipt of payment.
   5. Cards will only be ordered IF YOUR 2026 Dues HAVE BEEN PAID
