Custom Courtesy Cards

 

To order our 2024 Custom Courtesy Cards online, follow these directions.

1. Download the 2024 Courtesy Card Order Form
2. Print and complete the form by clicking this  2024 order form  OR– order online as explained in section 5
3. Click the link below which will take you to a GoDaddy payments page.
4. Enter the amount due based on the quantity ordered. 10 card minimum @ $25,00  Additional cards can be ordered in lots of 5 @ $2.50 for each additional card.
5. To order online without the printed form—-Click the payment page link below and—–Complete the NOTES section to spell out EXACTLY what you want on your card and verify the quantity ordered.  There is a 35 character limit.  Be sure to proof read your order, What you request is what you get.  It is your responsibility to make sure it is correct.
6. If you choose to use the paper form: Return the form either by US Mail… OR Scan it and send to secretarytreasurer@foplodge862.com
7. IMPORTANT NOTE:  YOU will  receive an emailed receipt of payment.
8. Cards will only be ordered IF YOUR 2024 Dues HAVE BEEN PAID
